A solução oftálmica Zymar é uma fluorquinolona com radical 8-methoxy que apresenta um duplo mecanismo de ação, o qual propicia vantagens bactericidas in vitro com relação a seus predecessores de terceira geração.(1) A atividade anti-bacteriana de Zymar in vitro resulta da inibição da girase do DNA (topoisomerase II) e da topoisomerase IV. A girase do DNA é uma enzima essencial que é envolvida na replicação, transcrição e no reparo do DNA bacteriano. A Topoisomerase IV é uma enzima que conhecidamente tem um papel chave na divisão do DNA cromossômico durante a divisão celular bacteriana.(3) Já que o mecanismo de ação de Zymar é diferente de outros antibióticos como aminoglicosídeos, macrolides e tetraciclina, a solução oftálmica Zymar pode ser ativa contra patógenos que são resistentes a estes antibióticos. Similarmente, bactérias resistentes a Zymar podem ser suscetíveis a estes agentes.
Indicações
Zymar é indicado para o tratamento de conjuntivite bacteriana causada pela ação suscetível de Corynebacterium propinquum,* Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus mitis,* Streptococcus pneumoniae e Haemophilus influenzae.
Reações Adversas
Os efeitos adversos mais freqüentemente relatados na população envolvida no estudo foram irritação conjuntiva, aumento da lacrimejamento, ceratite e conjuntivite papilar. Estes efeitos adversos ocorreram em aproximadamente 5% a 10% dos pacientes. Outras reações informadas, as quais ocorreram entre 1% e 4% dos pacientes foram quemose, hemorragia conjuntival, olho seco, lacrimejamento ocular excessivo, irritação ocular, dor ocular, edema de pálpebra, dor de cabeça, olhos vermelhos, redução da acuidade visual e alteração no paladar. Ao primeiro sinal de irritação ou reação alérgica, interrompa o uso imediatamente e contate o seu médico.
